Venous Leg Ulcer
Mr. HK is a 72 year old gentleman with a venous
ulcer over his left medial malleolus that had been
nonhealing for 3 years.
Case Study
Mr. HK wound was wet eczema and the area was very macerated (fig.1).
His pain level was at 4 on a scale of 1 to 10 with 10 being the worst pain that could be suffered.
A full vascular assessment was undertaken using Vascular Assist and this showed good wave forms and ABPI of 1.1 (left leg) and 1.08 (right leg).
Mr. HK was invited to take part in an evaluation of electrical stimulation (Accel-Heal) and his treatment in the WHC would then be free of charge. He agreed and he then had his legs soaked in warm water and dressed with a honey dressing and a foam adhesive.
He was then bandaged using a compression bandage, from toe to knee. This was the treatment he received on admission and this was continued for 4 weeks to provide standard care, with dressing changes as required, every 3 to 4 days. His wound was assessed using HDU on day 1 to provide a baseline and then reassessed on week 4 when AH was applied for the first time (fig. 2). Synapse was continued for a further 2 weeks with dressing changes every 48 hours.
At the end of the 2 weeks, assessment with HDU was undertaken and again at the end of a further 4 weeks (fig. 3). The wound was measured on computer with a start size of 3.82 cms down to 1.522 cms at week 10 with a further shrinkage 3 weeks post evaluation to 1.12 cms (fig. 4). The exudate was a moderate level throughout the treatment with yellow slough before treatment and granulation tissue apparent during treatment leading to closure after completion.
Conclusion
The graph below plots the healing line very well and demonstrates that the wound healed rapidly when Accel-Heal® was applied, but slowed once the stimulation was removed. His pain level dropped from 4 to zero during the application of Accel-Heal® and remained at zero.
